What is a Community Care Hub (CCH)?
CATCH is a community-centered nonprofit founded to bring together non-profits, community-based organizations (CBOs), medical providers, academia, and local government to integrate medical and non-medical care to meet the needs of our targeted populations to reduce healthcare costs, leverage resources, and improve quality of care.

How can my organization become a partner with CATCH?

- Review Process: CATCH evaluates potential partnerships based on alignment with its mission and goals.
- Sign Agreement: Once approved, you will sign a Master Service Agreement (MSA) outlining roles, responsibilities, and terms.
What is the onboarding process for CBOs?
The onboarding process for community-based organizations (CBOs) includes:
- Orientation: CATCH provides an overview of the Community Care Hub model, goals, and expectations.
- Needs Assessment: Organizations complete a survey to identify how they address health-related social needs (HRSN) like food insecurity, housing, and transportation.
- Training: CATCH offers training in collaborative workflows, reporting requirements, and technology use.
- Integration: Your services are integrated into the larger CATCH network, enabling referrals and collaboration with healthcare partners.
What are the benefits of partnering with CATCH?
There are many benefits to becoming a community-based organization partner of CATCH. These benefits include:
- As a partner, you’ll be able to cast a vote that influences decision-making by the CATCH Sub-Board on service delivery decisions. You have a “seat at the table” and are invited to provide input and feedback on CATCH operations, processes, and systems.
- As a partner, your organization can benefit when CATCH receives grant funding and contracts with healthcare organizations. You will be among the first to hear of new contracting opportunities, exclusive opportunities only to partners.
- Referrals for programs come through CATCH and are sent to agencies within the network. CATCH does the outreach and education to healthcare providers and seeks the referrals for you. We want to bring your agency business, clients, and reimbursements.
- Healthcare organizations prefer one point of contact. CATCH is this point of contact for healthcare organizations that contract with CATCH . CATCH bills the insurance companies and payers for the services your agency provides. We reconcile claims and denials and handle all the paperwork, process, and compliance that goes with billing.
- CATCH maintains a network-wide electronic health information system where all programs and services are documented. We are committed to developing systems which do not add to your administrative burden.
- CATCH provides training and education to our partners to ensure programs and services meet fidelity, compliance, and standards while growing capacity.
- We offer a robust Quality Assurance Program for all programs and services. This gives you peace of mind to know that we are dedicated to improving programs in all areas: delivery, promotion, referrals, efficiency, capacity, and more.
- CATCH’s knowledgeable team is available for support of your programs, services, and data integration in person, and via phone, email, or by text.
