About CATCH

Community Assistance and Transition Care of Houston (CATCH) has been a cornerstone of support for the Greater Houston area since 2011, formally establishing itself as a Community Care Hub (CCH) in 2023. Focused on addressing non-medical drivers of health, CATCH supports older adults, individuals with disabilities, and caregivers through a collaborative network of public health organizations and community-based entities.

 Our work ensures efficient access to services such as nutrition, transportation, and care coordination services, ultimately empowering clients to lead healthier, more self-sufficient lives.

Board

Executive Steering Committee

Advisory Members

Sets CATCH's strategic direction, ensures financial oversight, and upholds CATCH's mission and values. Board members are leaders from Houston Health Department, Ibn Sina Foundation, and Houston Health Foundation.

Consists of leaders and managers from Board-led organizations. Committee members may participate in the development of documents, assist with contract development, and make suggestions to the Board to help guide CATCH.

Advisory Members consist of individuals from CBOs that contract with CATCH and meet certain eligibility criteria. The Board votes on these members, and their role is to make recommendations to the Board and review CATCH's policies procedures
