GUIDE Program
What Is GUIDE?
The GUIDE Dementia Program is a Medicare initiative designed to improve care for people living with dementia and support their caregivers.
It provides a team-based approach that includes care coordination, caregiver support, and connection to community services.
Who May Be Eligible?
You may qualify if you:
- Have Traditional Medicare (Part A & B)
- Have a diagnosis of dementia (or may be evaluated during the process)
- Are not enrolled in Medicare Advantage
- Are not living in a long-term nursing facility
- Are not under hospice care
Note:
Participation in the GUIDE Dementia Program is voluntary. Completing a screening does not guarantee enrollment. Final eligibility is determined through Medicare program requirements and clinical review.
Support for Caregivers
GUIDE includes support for caregivers, such as:
- Education and training
- Ongoing support and guidance
- Access to respite services
Eligible participants may receive Medicare-covered respite services (up to approximately $2,500 per year) to provide temporary relief for caregivers.

If eligible and enrolled, the GUIDE program provides:
- Care coordination and ongoing support
- A dedicated care team
- Care planning and monitoring
- Referrals to community-based services
- Screening for health-related social needs
The goal is to help individuals remain safely in their homes and improve quality of life.
